TERMS OF REFERENCE
Purpose
The purpose of this assignment is to initiate an electronic data management system in selected health facilities in Islamabad Capital Territory that will include SRHR indicators.
Background
The SRHR Project/Initiative
In January 2019, World Health Organisation (WHO), signed a four-year donor agreement to implement the Initiative “Supporting country strategies to reduce maternal mortality and achieve Sustainable Development Goal (SDG) targets through a health systems approach” which is known as the Sexual Reproductive Health and Rights (SRHR) Initiative. The SRHR Initiative supports countries to make progress toward achieving universal health coverage (UHC) and meeting their SDG targets, particularly with reference to reduction of maternal mortality (SDG 3.1), universal access to sexual and reproductive health care services (SDG 3.7) and sexual and reproductive health and reproductive rights (SDG 5.6). The initiative focuses on supporting countries with a high burden of unintended pregnancy and unsafe abortion to reduce maternal mortality and morbidity from preventable causes.
Rationale
Pakistan is 5th most populous country in the world having sub-optimal progress on health indicators in general and sexual reproductive health in particular. The country has a young population with 60% of the population less than 30 years (Pakistan Demographic Health Survey (PDHS), 2018). This has a potential possibility of doubling the population in the immediate future.
There is high fertility rate (3.6%); low contraceptive prevalence (25% for modern methods); high abortion rate (29 per 1000 women of reproductive age); high adolescent birth rate (46 per 1000 population) (PDHS 2018); high maternal mortality (140 per 100,000 live births (Trends in maternal mortality 2019). According to the PDHS 2018, the country has a high perinatal mortality as well (neonatal mortality is 42 per 1000 live births and stillbirths is 47 per 1000 live births).
The Government has, since then, undertaken various efforts to gather critical information that aim to fill the current knowledge gaps in regard to SRHR related matters within the country. For example, a comprehensive situation analysis on status of UHC including SRHR. National/ Provincial laws governing various aspects of SRHR such as age at first marriage and abortion are being reviewed for gaps. These pieces of work are expected to highlight gaps and make recommendations for the way forward.
In order to support the pilot implementation of UHC benefit package in Islamabad as a model province there is need to support the component of Health information system. Considering the work load in health facilities, filling information in registers is a challenge and contributes to gaps in hospital data availability critical for decision making. It is against this background that the Ministry of National Health Services, Regulation & Coordination (MoNHSR&C) with the support of WHO, therefore, would like to strengthen electronic medical records system in selected facilities in ICT to facilitate collection of vital data in a an easier manner.
Objectives for the HIS Strengthening exercise
To introduce/strengthen the electronic data management system including dashboards on key UHC indicators including SRHR indicators in ICT.
Planned Timelines: A total of 85 days spread over the following time frame.

End date: 20th December 2020
Work to be performed
The APW holder(s)/ consulting firm is expected to liaise with the Federal Ministry focal points on Health Information systems and SRHR, Technical Officer SRHR and National Professional Officer (NPO) Reproductive Maternal New-born Child Adolescent Health (RMNCAH) in the WCO, DOH and Health facility HMIS focal points in undertaking this task.
S/he/They are expected to undertake the following:
Output 1: Prepare an inception report that provides timelines and methodology for the assignment.
Output 2: (a) Identify facilities for implementation of the pilot; (b) Identify personnel requirements for HIS implementation, (c) Identify systems requirements for HIS implementation
Output 3: Develop tools for HIS (including SRHR indicators) in consultation with Ministry/WHO
Output 4: Develop HIS for electronic data entry at first point of contact and all other points of contact at the facility.
Output 5: Develop online dashboard and link to National PHIS Dashboard (Enable GIS linkage with maps).
Output 6: Development of training modules for personnel at different levels who will be involved in HIS and enable future readiness of HIS and on-going user support.
Deliverables
1: Inception report providing timelines and methodology for the assignment including identified facilities, personnel and systems requirements.
2: HIS tools (including SRHR indicators) and HIS electronic data entry platform
3: Online dashboard linked to national PHIS dashboard that will enable GIS linkage with maps.
4: Training modules for Personnel involved in HIS at various levels.
Deliverables - Payment Schedule
First Deliverable: Signing of contract by 20th September - 0% payment
Second Deliverable: Submission of Tools for HIS (including SRHR indicators) in consultation with the Ministry and WHO by 20th October 2020 - 40% payment
Third Deliverable: Submission of HIS electronic data entry platform, online dashboard linked to National PHIS Dashboard and HIS training modules by 15th November 2020 - 40% payment
Fourth Deliverable: Final report on electronic HIS pilot in ICT by 31st November 2020 - 20% payment
